1. Change Request Form
Description: Permissions, access, or other IT-related changes
Approval: REQUIRES MANAGEMENT APPROVAL
Notes: Some requests may be automatically routed to the manager for approval before processing.
2. Position Change Request
Description: Employee role or department changes
Approval: REQUIRES MANAGEMENT APPROVAL
Notes: Manager review is required to confirm changes before implementation.
3. Termination Form
Description: Employee offboarding (HR only)
Approval: Reviewed by HR and management.
4. Onboard Form
Description: New hire setup or equipment (HR only)
Approval: Should only be submitted by HR.
How It Works
Submit your request using the appropriate form in Zendesk.
If the request requires approval, ITSD will initiate the approval process.
You will receive email updates when the request is approved, denied, or requires more information.
